Request Appointment

To schedule an appointment, you may send an email to or call/text 630-336-4442.

We make every attempt to respond to you promptly. We will call you to determine if telehealth / video is a good fit for your concern. At that time, we will send you a link to the Patient Portal and schedule your virtual appointment.

You can choose your provider or one will be assigned for you. You will then receive an e-mail with a link for the video appointment. Your provider will review the reason for the visit and your health history before your video call. We will ask you to complete the intake forms and insurance/credit card information prior to the appointment start time.

If lab work is necessary the provider will send an order to Quest Labs or a lab near you.

If a medication is needed, it will be prescribed by the provider. We do not prescribe any narcotics or other controlled substances.

We are in-network providers with Blue Cross Blue Shield of Illinois and we will file your claim for you.  If you are insured with any other company, we are considered out-of-network. In that case, you will pay for the visit at the time of service and upon request we will provide you with a superbill via e-mail that you may submit to your insurance company for reimbursement. We also accept cash payments or HSA/FSA.

In July 2021 Gov Pritzker signed into law HB 3308, which safeguards insurance coverage for telehealth services in IL through 2027. This law bars insurers from requiring patients to provide a reason for choosing a telehealth visit over an in-person consult. Telehealth services are reimbursed at the same rate as the equivalent in-person service in IL.
